Per Diem Registered Nurse

The per diem Registered Nurse works collaboratively with Care Design NY (CDNY) and Care Coordination/Health Home (CCO/HH) to facilitate holistic and integrated care management services for the people we support. The per diem Registered Nurse provides clinical expertise that supports quality improvement interventions and improved health outcomes. Such support operates under a whole-person approach to delivering care, integrating behavioral health, physical health, and developmental disability services, as well as other necessary supports and social services.

Responsibilities
  • Support healthcare initiatives in providing clinical support to complex people with I/DD.
  • Work collaboratively with the Nurse Manager of Healthcare Management to risk-stratify individuals for clinical assignments.
  • Carry a clinical assignment to support care managers with individuals who present as highly clinically complex.
  • Perform electronic chart reviews with a focus on quality improvement and continuity of care.
  • Documents clinical findings and recommendations in the electronic health record within the scope of their license.
  • Provide clinical guidance to support care managers in coordinating and accessing preventative and health promotion services.
  • Participate in complex care case reviews, clinical rounds, and other teams as needed to problem solve and assist those members most in need of a comprehensive and creative approach.
  • Meet all training requirements on time.
Qualifications
  • A Registered Nurse license to practice in New York state required.
  • A BSN is preferred.
  • Previous chart review experience required.
  • A minimum of 3 years of clinical experience in a hospital, skilled nursing facility, or subacute rehabilitation center.
  • Personal or professional I/DD experience is strongly preferred.
  • Experience in a managed care plan or health home is preferred.
  • Previous quality improvement experience preferred.
