Job description

Beloit Health System at Beloit Regional Hospice is seeking a compassionate and skilled Per Diem - Registered Nurse (RN) to provide exceptional end-of-life care to patients and support to their families. As a member of our Beloit Regional Hospice team, you will play a vital role in ensuring comfort, dignity, and quality of life for patients facing life-limiting illnesses.

The Registered Nurse (RN) at Beloit Regional Hospice is responsible for the assessment and care of patients and families in accordance with Beloit Regional Hospice policies and procedures. The Registered Nurse will work with the Interdisciplinary Team assuring that the patient and family needs are met, and care is provided consistently with federal and state regulations, Beloit Health System/Beloit Regional Hospice policies and procedures, and hospice standards of practice.
The Registered Nurse (RN) will work closely with and communicate regularly with the multidisciplinary team, including the primary care physician, regarding patient's condition.
The Registered Nurse (RN) will also:
  • Be attentive to physical, social, emotional, and spiritual aspects of patient/family experience, supporting directly or consulting with other team members to meet needs.
  • Document patient/family needs, goals, and interventions accurately in the medical record.
  • Document all patient encounters prior to end of shift for continuity of care.
  • Instruct and educate primary caregivers, family, volunteers, and employed caregivers to provide care as indicated.
  • Assess and admit patients, develop initial Interdisciplinary plan of care and complete paperwork in accordance with Beloit Regional Hospice policies and
  • Communicate in a positive, responsive, cooperative manner with patients, families, team members, and all others you encounter as a representative of Beloit Regional Hospice.
  • Anticipate and meet patient and family needs. Participate in agency in-services and community education opportunities as requested to promote

Job Requirements
  • Graduate of accredited nursing program. BSN preferred.
  • Current RN license in Wisconsin or Illinois, with the ability to obtain a RN license in the opposite state. Must apply within 30 days of hire.
  • Valid driver's license and reliable vehicle.
  • Ability to work independently with minimal instruction.
  • Maintain current knowledge of state and federal hospice regulations, and the RN Scope of Practice for Wisconsin and Illinois.
  • Maintain ethical conduct and keep confidential personal and medical information about patients.
  • Will assist with call schedule
