RN Field Staff (Per Diem)
Southwoods Health at Home, a division of Southwoods Health established in 2022, is seeking a compassionate and skilled Per Diem RN Field Staff Nurse. This role is vital in providing high-quality, patient-focused care within the home setting. As a member of our field staff, you will manage an assigned caseload of patients from admission through discharge, ensuring a seamless and personalized healthcare experience.
Essential Duties:
  • Comprehensive Care Coordination: Manage a designated caseload, assessing patient needs and developing individualized care plans from the start of care through discharge.
  • Collaborative Practice: Participate as a key member of a multidisciplinary team to ensure optimal patient outcomes and positive experiences.
  • Patient Advocacy amp; Monitoring: Closely monitor patient status, carry out ordered interventions, and adjust care plans as patient needs evolve.
  • Clinical Documentation: Accurately and articulately document all clinical findings, interventions, and patient responses in accordance with regulatory standards.
  • Safety amp; Compliance: Ensure all clinical processes maintain compliance with regulatory agencies (including OASIS requirements where applicable).
  • Communication: Keep the multidisciplinary team, patients, and families updated on progress toward established goals.
  • Resource Stewardship: Utilize clinical resources effectively, considering cost-containment without compromising the safety or quality of care.
Qualifications:
  • Licensure: Active Registered Nurse (RN) license in the State of Ohio.
  • Certification: Current AHA BLS certification.
  • Education: Bachelor of Science in Nursing (BSN) is preferred.
  • Clinical Experience: Prior experience in home health care is highly desirable.
  • Mobility: Valid Driver’s License and proof of current automobile insurance; ability to travel to patients’ homes within the assigned area.
  • Core Competencies: Strong critical thinking, problem-solving, and effective communication skills.
