In this full-time remote position, you will be instrumental in delivering comprehensive mental health services to clients, particularly within the hospice setting. You will work closely with other healthcare professionals, including registered nurses and care coordinators, to provide holistic patient care. Typical work hours are Monday to Friday from 9 AM to 5 PM, allowing you to enjoy evenings and weekends.

• Conduct thorough assessments, including complete mental status examinations, to gauge patient conditions and recognize changes in their mental state.
• Provide intensive counseling and psychotherapy to individuals and families, focusing on emotional support and treatment needs.
• Develop and adapt individualized treatment plans that align with patient goals, actively involving clients in their care strategies.
• Establish and nurture collaborative relationships with a multidisciplinary team to ensure a comprehensive approach to care, enhancing patient outcomes.
• Offer crisis intervention services as needed, providing immediate emotional support during critical moments.
• Engage in continuous professional development to remain current with mental health practices and hospice care regulations.

Your primary focus will be on patients receiving hospice care, many of whom are facing end-of-life challenges. You will provide emotional and psychological support for patients navigating complex feelings surrounding their conditions while also addressing family dynamics and emotional needs. This role is crucial in ensuring that our patients experience a dignified and peaceful transition.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• Valid California Licensed Clinical Social Worker (LCSW) credentials.
• Strong communication and interpersonal skills, with the ability to build rapport with diverse patient populations.
• A compassionate and empathetic demeanor, demonstrating genuine concern for patient wellness.
• Ability to work both independently and collaboratively within a multidisciplinary team.

• At least 2 years of experience in mental health counseling, specifically in hospice or palliative care settings.
• Familiarity with electronic health records (EHR) and telehealth platforms for client management.
• Experience in case management and working with external resources.

This position offers a competitive annual salary ranging from $110,000 to $130,000, commensurate with experience.
