• Conduct thorough initial assessments and ongoing evaluations to formulate individualized treatment plans tailored to each patient’s unique psychological and emotional needs.
• Implement evidence-based therapeutic interventions, such as cognitive-behavioral therapy and solution-focused brief therapy, to support client well-being and recovery.
• Maintain detailed and accurate clinical documentation, ensuring compliance with ethical standards and patient confidentiality regulations.
• Collaborate effectively with a dedicated treatment team, including psychiatrists, nurses, and case managers, to ensure comprehensive and holistic care for all patients.
• Educate and empower patients and their families about mental health concerns, coping strategies, and available resources.

You will work primarily with patients at the hospice center who are experiencing significant emotional and psychological distress due to terminal illness and life-limiting conditions. Your approach will be informed by a deep understanding of grief and bereavement processes, enabling you to offer compassionate support to both patients and their families during one of the most challenging times in their lives.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Washington.
• At least 2 years of clinical social work experience, ideally in mental health or hospice care.
