AdvisaCare is seeking a dedicated and compassionate Per Diem Hospice Registered Nurse to join our team. As a Hospice RN, you will play a vital role in providing exceptional end-of-life care to patients and their families. As a Per Diem Hospice RN, you will be responsible for assessing patients' needs, developing and implementing personalized care plans, and providing compassionate support to patients and their families. This role requires strong clinical skills, excellent communication abilities, and the ability to work effectively in a team environment.

Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs
  • Develop and implement personalized care plans in collaboration with a multidisciplinary team
  • Administer medications, provide symptom management, and monitor patients' conditions
  • Educate patients and their families on disease management, end-of-life care options, and available resources
  • Coordinate and facilitate communication between patients, families, and the hospice team
  • Provide emotional and psychological support to patients and their families
  • Ensure accurate and timely documentation of patient care activities

Requirements

  • Current and unrestricted Registered Nurse license in the state of Arizona
  • Minimum of [1] years of experience in hospice or palliative care or 1-2 years of critical care
