The Physical Therapist position is open in our Apple Valley location and surrounding areas. This is a Per-Diem position during our day shift.
Requirements of the Physical Therapist:
• Graduate of a program of physical therapy approved by the APTA and the
• Council of Medical Education of the AMA.
• Current licensure for practice in the State of California.
• At least two year of experience in a clinical setting.
Physical Therapist Duties Include:
• Implements current physical therapy practice.
• Ensures quality and safe delivery of home health and hospice care services.
• Develops a Plan of Care based on the patient's medical needs and discusses with the Case Manager and/or Clinical Manager. Revises plan as necessary for appropriate utilization of services and discusses changes with members of the Interdisciplinary Team.
• Promotes effective written/verbal communications.
• Uses equipment and supplies effectively and efficiently.
• Identifies need for intra-agency referrals as well as inter-agency referrals.
• Promotes education for clients, client families and the community.
• Participates in personal professional growth and development.
• Conferences with Case Manager and/or team members during the patient's course of treatment.
• Provides inservice to other staff members as requested.
