  • Develop, review, and update individualized care plans

  • Supervise CNAs and provide client-specific instruction

  • Collaborate with physicians, families, hospitals, and other providers

  • Accompany clients to medical appointments when needed

  • Support hospital discharges and safe transitions home

  • Provide medication management, wound care, and other skilled nursing services

  • Document clinical updates accurately and communicate concerns promptly

Who We’re Looking For

Our ideal nurse is an experienced servant leader who combines strong clinical judgment with compassion, professionalism, and resourcefulness. You should be comfortable making independent decisions in a home setting while keeping clients, families, and the care team informed.

Qualifications
  • Active, unrestricted Rhode Island RN license

  • Current CPR certification

  • Five or more years of RN experience

  • Five or more years of care planning, case management, or patient-navigation experience

  • Experience in home care, rehabilitation, long-term care, assisted living, hospice, or a related setting preferred

  • Consistent weekday business-hour availability

  • Strong assessment, documentation, and communication skills

  • Valid driver’s license, reliable vehicle, and current auto insurance

  • Ability to travel throughout Providence, Kent, and Washington counties

  • Ability to lift up to 50 pounds and assist with client mobility when required

Benefits
  • Premium pay reflecting your clinical experience

  • Flexible weekday scheduling

  • 401(k) with company match

  • Free uniforms

  • Clinical autonomy with reliable leadership support

  • Ongoing professional development

  • Opportunities for expanded clinical and leadership responsibilities
