Job Description

Evaluates, plans, directs and administers individualized therapeutic treatment programs aimed at the restoration or improvement of function, or the prevention of disability. Writes evaluations, progress notes/reports, discharge summaries and maintains patient files. Counsels patients and/or family members regarding Rehabilitation diagnosis, plan of care and prognosis. Maintains current knowledge of regulatory/legislative policies as they apply to the profession of Physical Therapy and adheres to same.

Qualifications

Graduate of Accredited Physical Therapy Program, Current NJ State License in Physical Therapy. CPR certification within first 3 months of hire. FIM certification to be taken following employment and training at Wayne, then certification must be taken through the rehab program (Uniform Data System for Medical Rehab-UDSmr). Valid driver's license required and comply with Qualifications to Drive a System Vehicle as well as all policies as outlined in the SJHS Fleet Management & Safety Program for EIP (Early Intervention Program) employees only. Effective January 2019, St. Joseph's Health requires all employee certifications for Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) to be granted through the American Heart Association.
