Job description

SUMMARY

As a Nurse Practitioner, your superior interpersonal and empathetic skills, as well as your comprehension of diagnostic procedures greatly assists in managing and delivering quality treatment plans.


STATUS: Per Diem


DEPARTMENT: Palliative Care


LOCATION: Remote


SCHEDULE: Days


ATTRIBUTES



  • Experience as an NP in palliative care, hospice or relevant field is required


  • Strong customer service and communication skills


  • Compassionate, warm and patient focused


  • Must be able to work in a stressful environment and take appropriate action


  • Critical thinking skills, decisive judgment and the ability to work with minimal supervision


  • Exceptional documentation skills and professional behavior



RESPONSIBILITIES

  • Patient Care. Provide primarily remote care to palliative care patients, responding to triage needs and more urgent concerns through video visits.


  • Patient Satisfaction. Communicate with patients and families with empathy and understanding, providing education, support, and guidance related to their care.


  • Care Coordination. Collaborate with the palliative care team to assess patient needs, provide appropriate recommendations, and coordinate care.


  • In-Person Coverage. Complete required in-person training and provide occasional in-person coverage for vacation and sick-call needs.



EDUCATION:

  • MS: Nursing (Required)


PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
