Per Diem Hospice RN Case Manager

Location: Plymouth, MA

Job Summary:

Hospice Services of Massachusetts is seeking a compassionate and experienced RN Case Manager to join our team! As a Hospice RN Case Manager, you will be responsible for planning, organizing, and directing hospice care to meet the needs of patients and their families in their homes and communities. We offer a competitive salary range of $42 to $52 hourly, depending on experience, and an opportunity to make a lasting difference in the lives of our patients.

Responsibilities:

  • Supervise hospice aides

  • Conduct comprehensive assessments of patients and families to determine hospice needs

  • Develop and implement individualized plans of care

  • Coordinate patient care services

  • Provide professional nursing care and counsel patients and families

  • Participate in ongoing education and quality improvement initiatives

Requirements:

  • Graduate of an accredited school of nursing

  • One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care

  • Current licensure in the state and CPR certification

  • Bachelor's degree, with one year of hospice care experience preferred
