As a Per Diem Hospice RN, you will be responsible for assessing patients' needs, developing and implementing personalized care plans, and providing compassionate support to patients and their families. Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs
  • Develop and implement personalized care plans in collaboration with a multidisciplinary team
  • Administer medications, provide symptom management, and monitor patients' conditions
  • Educate patients and their families on disease management, end-of-life care options, and available resources
  • Coordinate and facilitate communication between patients, families, and the hospice team
  • Provide emotional and psychological support to patients and their families
  • Ensure accurate and timely documentation of patient care activities
Requirements
  • Current and unrestricted Registered Nurse license in the state of Arizona
  • Minimum of [1] years of experience in hospice or palliative care or 1-2 years of critical care
