Your Role
  • Assess patients' physical, psychosocial, environmental, and educational needs

  • Develop, implement, and update individualized plans of care

  • Manage an assigned caseload from admission through discharge

  • Provide skilled nursing care and patient education during home visits

  • Coordinate care with physicians, caregivers, therapists, and clinical leadership

  • Identify and communicate changes in condition or safety concerns

  • Complete accurate and timely clinical documentation

  • Advocate for each patient's goals, independence, and quality of life

What You'll Bring
  • Current, unrestricted Massachusetts RN license

  • At least one year of clinical nursing experience

  • Associate degree in Nursing required; bachelor's degree preferred

  • Home health or hospice experience preferred

  • Homecare Homebase experience preferred

  • Dependable vehicle, valid driver's license, and current auto insurance

  • Willingness to travel throughout the Northampton territory

  • Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
