Role:The Pediatric Hospice Liaison serves as the primary relationship-builder and referral coordinator between pediatric hospice services and hospitals, physicians, clinics, families, and community organizations. The role focuses on educating healthcare providers and families about pediatric hospice and palliative care services, supporting admissions, cultivating pediatric hospice referrals, and supporting continuity of care for children with life-limiting illnesses.
Core Responsibilities
- Develop and maintain referral relationships with pediatricians, hospitals, NICUs, PICUs, specialty clinics, social workers, and case managers.
- Educate healthcare professionals and community partners about pediatric hospice and palliative care services, eligibility, and benefits.
- Serve as the main point of contact for incoming referrals and coordinate the intake/admission process.
- Meet with patients and families to explain hospice services, answer questions, and provide emotional support during care transitions.
- Collaborate with interdisciplinary teams including nurses, physicians, social workers, chaplains, and therapists to ensure coordinated patient care.
- Conduct outreach activities, presentations, and community engagement events to increase awareness of pediatric hospice services.
- Track referral activity, maintain accurate documentation, and support census growth goals.
- Ensure compliance with healthcare regulations, HIPAA requirements, and organizational policies.
- Build partnerships with schools, support agencies, and community resources serving children and families.
- Participate in strategic planning and marketing initiatives to expand pediatric hospice programs.

Experience Requirements
At least three years experience working in pediatric healthcare, hospice, palliative care, case management, or healthcare marketing preferred. Knowledge of hospice regulations, insurance processes, and referral coordination preferred.
Education Requirements:
Bachelor's degree in nursing, social work, healthcare administration, or related healthcare field.
Required Certificates and/or Licenses:
Active state licensure in nursing, social work, or certified child life (if applicable).
Overall Skills:
The ability to demonstrate superlative oral and written communication skills, in English, as well as ability to organize and manage projects within defined timeframe and budget; work both independently and as part of a team; demonstrate excellent interpersonal and collaborative; demonstrate organizational skills with ability to delegate and follow up; manage multiple tasks and stressful situations with a positive attitude; demonstrate a strong work ethic and attention to detail; high degree of accuracy in typed work; excellent proofreading skills; learn patient health information systems; demonstrate excellent interpersonal and communication skills necessary to positively influence and interact with the public, volunteers, co-workers and donors.
