At Peace Healthcare Agency is seeking a dedicated and compassionate ODP/IDD Program Coordinator to join our team. In this pivotal role, you will act as a primary advocate and manager for individuals with intellectual and developmental disabilities. You will be responsible for ensuring that the services provided align with the standards set by the Office of Developmental Programs (ODP) and that every individual receives high-quality, person-centered care.

Key Responsibilities

  • Case Management & Advocacy: Oversee the development, implementation, and ongoing monitoring of Individual Support Plans (ISPs). Ensure these plans accurately reflect the individual's needs, preferences, and long-term goals.

  • Regulatory Compliance: Maintain thorough, up-to-date documentation in accordance with Pennsylvania ODP regulations and agency policies. Prepare for and participate in licensing audits and internal quality reviews.

  • Stakeholder Collaboration: Serve as a central point of contact for families, support coordinators, direct support professionals, and medical providers to ensure coordinated care.

  • Crisis Intervention & Support: Respond promptly to critical incidents, ensuring the safety and well-being of the individuals we serve. Facilitate proactive problem-solving to mitigate ongoing risks.

  • Team Leadership: Provide guidance and support to direct care staff regarding implementation of support plans and best practices for IDD care.

Qualifications

  • Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a closely related field.

  • Experience: Minimum of 2–3 years of experience in human services, specifically working within the IDD population and the Pennsylvania ODP system.

  • Technical Skills: Proficiency in HCSIS (Home and Community Services Information System) and standard electronic record-keeping software.

  • Soft Skills: Strong communication, organizational, and conflict-resolution skills. A warm, patient, and professional demeanor is essential.

  • Requirements: Valid driver’s license and reliable transportation for regional travel. Ability to pass required background checks and clearances.
