Nurse Practitioner (NP) / Physician Assistant (PA) - Post-Acute, East Valley, AZ
Location: Mesa, AZ (Primary) Some coverage in Chandler
Setting: Skilled Nursing & Assisted Living Facilities
Schedule: Full-time, weekdays on-site 9 AM - 5 PM, plus weekend rotation once or twice per month
Employment Type: 1099 Independent Contractor
Compensation: Productivity-based pay with potential to earn up to $300,000 annually
About the Opportunity
We're hiring a full-time Nurse Practitioner or Physician Assistant to provide ongoing care within post-acute and long-term care settings across Mesa and Chandler.
This position offers steady weekday hours, autonomy, and a manageable patient load in a supportive clinical environment.
Providers typically see 15-16 patients per day in Skilled Nursing and Assisted Living communities, focusing on established, stable residents and new admissions as needed.
Key Responsibilities
  • Provide direct, in-person care at assigned SNF and ALF facilities
  • Maintain an on-site presence daily
  • Participate in weekend rotation once or twice per month
  • Manage 15-16 patient visits per day, including new and follow-up appointments
  • Respond to nursing staff calls related to your assigned patients
  • Conduct assessments, adjust medications, and manage chronic conditions
  • Collaborate with facility staff and interdisciplinary teams

  • Active, unrestricted Arizona NP or PA license
  • Graduate of an accredited U.S.-based NP or PA program
