All Metro Health Care, a Modivcare Personal Care Service, is seeking a Per Diem RN Supervisor - Home Care to support our clients and caregivers in the Corning, NY area. This is an ideal opportunity for an experienced RN looking for flexible, per diem, field-based work while maintaining a strong clinical and leadership presence in home care.

  • Supervisory / Post-Hospital Visit: $35-$60 per visit

Schedule & Location
  • Per diem role requiring availability 1-2 days per week

  • Office visits are limited to orientation, and meetings as needed
What You'll Do
  • Conduct on-site supervisory and teaching visits to evaluate caregivers
  • Provide clinical orientation and ongoing guidance for assigned home care cases
  • Complete initial nursing assessments and reassessments
  • Participate in training and in-services for field staff
  • Collaborate with patients, caregivers, and physicians to assess needs and adjust care plans
  • Ensure accurate, timely clinical documentation and compliance
  • Monitor patient condition changes and communicate appropriately
What We're Looking For
  • Registered Nurse (RN) - active NY license
  • CPR Certified
  • Minimum 1 year of home care experience
  • 1 year of supervisory or management experience
  • Valid NYS driver's license, auto insurance, registration, and reliable vehicle
