Job description

PAX Health is a leading mental and behavioral health organization dedicated to providing comprehensive and compassionate care to individuals facing mental health challenges. The Therapist at PAX Health is responsible for delivering high quality, evidence based clinical services to patients and their families within PAX Health's outpatient setting. This includes individual, group, family, and/or couples therapy tailored to each patient's needs and level of care.
They play a key role in assessment, diagnosis, treatment planning, ongoing care coordination, and discharge planning. In partnership with clinical leadership and the multidisciplinary treatment team, the Therapist ensures services are delivered in alignment with organizational standards, regulatory requirements, ethical guidelines, and evidence based practices, while maintaining a patient centered and culturally responsive approach to care.
Responsibilities:
  • Receive, review, and respond promptly to assigned intake referrals
  • Conduct biopsychosocial assessments and support diagnostic formulation
  • Establish diagnoses and develop individualized treatment plans appropriate to level of licensure and supervision requirements
  • Provide direct clinical services, including individual, group, family, and/or couples therapy
  • Maintain an active caseload appropriate to experience level and licensure status
  • Independently manage treatment planning and clinical decision making when fully licensed
  • Participate in individual and group supervision in accordance with licensure requirements when provisionally licensed
  • Monitor patient progress and adjust treatment approaches based on clinical need and outcomes
  • Support discharge and aftercare planning to ensure continuity of care
  • Provide case management services and coordinate referrals to external resources when appropriate
  • Maintain accurate, timely, and compliant documentation within the Electronic Health Record (EHR)
  • Ensure coordination and integration of services documented across providers and disciplines
  • Collaborate with prescribers, including the Medical Director, and multidisciplinary care teams to support patient outcomes
  • Participate in treatment team meetings, clinical consultations, staff meetings, trainings, and professional development activities
  • Comply with all local, state, and federal confidentiality and regulatory requirements
  • Deliver culturally responsive, patient centered care aligned with PAX Health values
  • Abide by PAX Health code of ethics and model values
  • Perform other duties as needed within the scope of the role
Requirements
  • Master's degree in Social Work, Counseling, Psychology, or a related field
  • Active and unrestricted clinical license or provisional license in practicing jurisdiction (LCSW, LPC) (Pennsylvania)
  • Fully licensed clinicians must be able to assess, diagnose, and treat patients independently without supervision
  • Strong clinical skills in assessment, treatment planning, therapeutic interventions, and evidence based modalities
  • Interest and/or experience in modalities such as CBT, DBT, ACT, trauma informed care, or similar approaches
  • Experience working with diverse patient populations and a range of mental health conditions preferred
  • Strong communication and interpersonal skills
  • Strong organizational, time management, and documentation skills
  • Proficiency with EHR systems
  • Ability to collaborate effectively within a multidisciplinary team
  • Commitment to ethical practice, professional growth, supervision, and ongoing learning
  • Commitment to delivering high quality, patient centered care
  • LCADC or CADC is a plus
