Job Description

Provide quality patient safety monitoring services under supervision of a licensed professional. Proactively maintain a safe environment for the patient by providing 1:1 continuous monitoring/reliable video monitoring and reporting potential unsafe and harmful behaviors.

Experience

  • No minimum experience required

Preferred Qualifications

  • Experience working in a healthcare setting
  • Academic healthcare-related courses

Education

  • High school diploma or equivalent

Certification Summary

  • Basic Life Support (BLS) within 30 days of employment or at time of offer if working in the Emergency Department

Knowledge, Skills, and Abilities

  • Must have strong communication and interpersonal skills
  • Must be able to successfully pass the Basic Windows Skill Assessment at 80% or higher rating within 30 days of employment

Job Responsibilities

  • Maintains a safe environment for the patient through 1:1 continuous monitoring that includes remaining within arm's length and facing the patient at all times, including during bathroom and shower activities.
  • Complies with all related policies requiring a patient safety supporter role (i.e., suicide policy, restraint policies).
  • May perform/document components of safety monitoring and general care needs (eating, hydration, and/or elimination).
  • Performs reliable video monitoring in areas that video monitoring is available and/or performed.
  • Communicates and interacts with patients and family in a therapeutic and respectful manner, with intent to minimize escalation of behaviors.
  • Monitors for behavior changes and escalates any untoward events (i.e., behavior changes, violent behavior) to appropriate staff, and responds appropriately to direction given by healthcare team members.
  • Accompanies patient to medical procedures to maintain 1:1 continuous monitoring requirement.
  • Accurately documents data collected and behaviors observed on assigned patient in EMR, according to establish time frames.
  • Participates in transfer of care (patient safety supporter to patient safety supporter, patient safety supporter to nurse, patient safety supporter to other care team members); communicates any special considers, care priorities, and support needs as appropriate.
  • Maintains patients' rights, including the privacy and security of all confidential and protected health information.
  • Demonstrates all competencies of the job: a.Initial Competencies b.Ongoing, annual competencies c.Age specific and cultural competencies.
  • Accompanies patients to various locations on the unit and hospital.
  • Stocks supplies.
  • Assists patient with activities of daily living.
  • Transports labs, medications, blood products, and other items as requested or assigned.
  • Any other duty as reasonably requested by unit leadership.
