Wellness Nurse (LPN) Role Overview:

As a Wellness Nurse (LPN), you’ll play a vital role in delivering high-quality care to our residents. You will administer medications, support daily wellness routines, supervise care staff, and help ensure each resident’s health and dignity are prioritized.


Wellness Nurse Key Responsibilities:

  • Provide direct nursing care and supervise medication care specialists.

  • Administer medications, treatments, and procedures per physician’s orders.

  • Document resident care and update care plans as needed.

  • Monitor for changes in residents’ health and communicate updates to physicians and families.

  • Assist with resident move-ins and orientation.

  • Provide leadership and guidance to care staff, including performance evaluations and coaching.

  • Participate in emergency response procedures and ensure safety standards are upheld.

  • Maintain accurate resident records and ensure compliance with community policies and state regulations.

  • Promote resident independence, wellness, and participation in daily activities.

  • Foster a positive, respectful environment for residents, families, and team members.
     

Wellness Nurse Qualifications:

  • Active LPN license in the state of employment.

  • Minimum 1 year of nursing experience; experience with Alzheimer’s/dementia preferred.
