Job description

We are currently seeking a dedicated and Bilingual (Spanish/English) Patient Intake Coordinator to join our team at our Bronx location. As a Patient Intake Coordinator, you will play a crucial role in facilitating the enrollment process for individuals seeking home care services.

Responsibilities:

  • Explain program benefits, and conduct initial eligibility (Medicaid and Managed Long-Term Care) reviews and screenings for individuals interested in participating for home care services.

  • Conduct outbound calls to an assigned census of 40-50 active patient cases and answer incoming calls with a goal of 30+ monthly conversations converted into qualified leads.

  • Coordinate and manage the intake process, including gathering necessary documentation and information.

  • Collaborate with healthcare professionals, caregivers, and clients to ensure smooth onboarding and enrollment procedures.

  • Maintain accurate and confidential records of all intake activities and participant information.

  • Provide exceptional customer service by educating clients and caregivers on program guidelines and procedures and addressing inquiries, and concerns, throughout the intake process.

Requirements:

  • Fluency in both English and Spanish (written and verbal) is essential.

  • Experience in a similar role in Intake or Onboarding home care clients.

  • Strong organizational skills with attention to detail.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and collaboratively in a team-oriented environment.

  • Proficiency in basic computer applications, (Salesforce a plus) and data entry.
