What You'll Do
  • Lead and coordinate patient care in collaboration with physicians
  • Perform assessments (including OASIS) and develop care plans that truly fit the patient
  • Supervise and support LPNs and Home Health Aides during visits
  • Communicate closely with families and interdisciplinary team members
