On Call Registered Nurse (7 days on, 7 days off)

At Ancora Home Health & Hospice we are expanding our Home Health & Hospice Nursing Teams and are seeking a dynamic and experienced individual for the role of On Call Registered Nurse who shares our desire to make healthcare better for everyone.

COMPENSATION: $40,000/base salary + pay per visit

STATUS: On Call

SCHEDULE: 7 days on and 7 days off.

(Monday-Thursday 5pm-8am and then Friday 5pm-Monday 8am.)

Job Summary As a RN Case Manager at Ancora, you will play a crucial role in delivering exceptional home health and end-of-life care to our patients. Your compassionate approach will ensure comprehensive physical, social, and psychological assessments and your friendly and supportive demeanor will actively engage patients, families, and the rest of our Interdisciplinary Group in developing comprehensive care strategies and plans. With your attention to detail, you will efficiently coordinate patient care delivery, manage resources, and maintain meticulous patient records. Your positive collaboration with other team members, patients, and their families, and proactive initiation of referrals will create a holistic care experience. Position Requirements

  • Schedule: This role operates on a 7 days on, 7 days off rotation throughout the year.
  • Minimum Commitment: Must work a minimum of 15 days per month, including 2 weekends.
  • Responsibilities: While on call, you will be expected to handle all patient needs, including:
    • Medically necessary scheduled visits.
    • PRN (as-needed) visits.
    • New patient admissions.
    • Responding to and managing death calls.

This structure ensures comprehensive care delivery and continuity for our patients during your on-call periods.

  • Current Registered Nurse (RN) license in the state of practice
  • Home Health / Hospice experience as a Registered Nurse preferred
  • Ability to utilize full range of movement to transfer patients
  • Effective verbal & written communication & time management skills
  • Valid Driver's License (clean record) & reliable transportation.
  • Proof of current auto insurance & registration
  • Valid AZ Fingerprint Clearance Card (we can help)
  • Current CPR Certification (we can help) & Negative TB Test (we can test)
