RESPONSIBILITIES:
Patient Care:
- Be responsible for nursing functions related to the treatment programs in Project Turnabout.
- Assist the patients to maintain and improve their physical status.
- Prepare and maintain the health records of patients, including admission paperwork (both written and electronic) and remain within expected timelines per licensure requirements
a. Nursing Assessment completed upon initial assessment/intake
b. Stabilization plans completed within 12 hours of admission for all detox patients
c. 4-12 Hour Follow-Up documentation done 4-12 hours after admission for all detox patients
d. Daily progress notes to address stabilization services that are listed in each patient’s stabilization plan.
e. Vitals and withdrawal assessment scales performed at least every 4 hours on each patient or as necessary per patient status and charted prior to leaving shift
f. All medications are passed for each med pass time scheduled during shift
- Know and understand statutes as a responsible staff person. Each responsible staff person must know and understand the requirements of this chapter, sections 245A.65, 253B.04, 253B.051, 626.557, 626.5572, and chapter 260E. In a clinically managed program, the responsible staff person must be a licensed practical nurse employed by or under contract by the license holder. In a medically monitored program, the responsible staff person must be a registered nurse, program director, or physician.
- Delegate nursing duties to staff while professionally remaining within the appropriate scope of practice.
- Maintains and adheres to established infection control/employee health program
- Participate in developing/implementing individual treatment plans for patients.
- Correspond with Doctors, Physician’s Assistants and any other health care professionals on a regular basis.
- Dispense medication as prescribed.
- Accurately assess detoxification withdrawal according to CIWA/COWS assessment scales, maintaining close monitoring of all detox patients until medically cleared to begin treatment or to be discharged/transferred from facility.
- Implement standing orders when medically necessary and follows correct protocol(s)
- Coordinates emergency medical care for medical and psychiatric emergencies
- Perform individual case management as requested
- Document medical aftercare plans when necessary
- Perform count of controlled medications at the beginning of each shift (Only for nurses delegated to be key holders ONLY–exemptions would be HPSP restrictions)
- Accept and admit all detox patients appropriate for facility based on “Admission Criteria” policy and state regulations
- Perform complete physical assessment on all admissions
- Obtain 72 hour hold orders from law enforcement and/or medical doctors
- Administer dietary orders and education as needed and ordered
- Take all professional phone calls as needed, including after hours and on weekends.
- Perform continual follow-up on all patient cases.
- Review medical records for potential admissions in order to determine appropriate placement
- Communicate patient needs with counseling and/or other appropriate staff
- Maintains a “patient-first” and team-oriented focus, communicating with other staff on the unit to ensure the best outcomes for patients at Project Turnabout.
