PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Cordova, Tennessee.

Position Overview:
Seeking a compassionate and patient focused Registered Nurse to provide case management and skilled hospice care in the home setting. This role is centered on delivering high quality end of life care, ensuring patient comfort, managing symptoms, and supporting patients and families throughout the hospice journey.

Schedule & Work Details:

  • Location: Shelby, Fayette, and Tipton Counties, TN
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hours: 40 hours
  • On Call/Weekends: Not required
  • Productivity Expectation: 6 visits per day
  • Pay Rate: $51 to $53 per hour
  • Mileage Reimbursement: Yes, reimbursed between patient visits

Key Responsibilities:

  • Provide case management for hospice patients, ensuring comprehensive and coordinated care
  • Perform routine hospice visits focused on symptom management and patient comfort
  • Assess patient condition and adjust care plans in collaboration with physicians and interdisciplinary team members
  • Prepare for and participate in Interdisciplinary Team (IDT) meetings
  • Educate patients and families on disease progression, medications, and end of life care expectations
  • Maintain accurate and timely documentation using Homecare Homebase (PointCare)
  • Communicate effectively with care team members to ensure continuity and quality of care
  • Deliver compassionate support to patients and families during end of life care

Required Qualifications:

  • Active Registered Nurse (RN) license
  • Recent hospice nursing experience required
  • Experience with Homecare Homebase (HCHB/PointCare)
  • Strong clinical assessment and symptom management skills
  • Ability to work independently and manage a caseload
  • Valid driverโ€™s license and reliable transportation
