Job description

Futures Recovery Healthcare is a state-licensed, Joint Commission accredited behavioral healthcare provider located in Tequesta, Florida of Palm Beach County. At Futures, we are committed to treating the whole person, not just the behaviors. We have a holistic approach that is patient-centered. Our medical, clinical, resident assistant, admissions, outreach, executive, culinary, alumni, and intake teams all work together to meet a patient where they are and allow them to heal and recover in a safe and nurturing environment, which is essential for whole healing and life-long recovery.

The Case Manager provides a strong therapeutic relationship to the patient, collaborating with the Primary Therapist in identifying, establishing and communicating the clinical recommendations for continued care. Case Manager will work with the patient coordinating the aftercare/continuing care recommendations provided by the clinical team.   The Case Manager will provide support to the patient in respect to legal and any other identified documentation. Case Manager is to maintain ongoing communication and relationships with referral sources as well as invested parties.

Duties and Competencies:

  • Identifies and assesses case management needs and provides discharge planning to assure that the patient progresses through the continuum of care and is provided with an aftercare plan based on recommendations provided by clinical, medical and psychiatric providers.
  • Facilitate group therapy in accordance with license requirements, AHCA, The Joint Commission standards and Futures of Palm Beach policies
  • Develop and document a comprehensive clinical Discharge Plan
  • Communicates with referents.
  • Coordinates the facility activities concerned with case management and discharge planning
  • Provide didactic/educational lectures
  • Effectively communicates relevant clinical information to the clinical team, patient and family
  • Actively participates in treatment team meetings, providing relevant patient case assessment
  • Documentation the progress and/or regression of the patient in an accurate and timely manner
  • Respond to and assist with crisis intervention and conflict resolution
  • Demonstrates ability to respond to feedback and supervision from supervisor and peers
  • Adheres to Futures of Palm Beach, LLC policies and procedures
  • Adheres to departmental goals, objectives, standards of performance, and policies and procedures
  • Maintain appropriate boundaries, attendance, professionalism, and ethical behavior with patients, families, and other professionals
  • Uphold strict patient confidentiality in accordance with Title 42 CFR, Part 2 and HIPAA and Futures of Palm Beach Policies

Qualifications:

  • Bachelor’s degree from an accredited school in behavioral sciences, counseling, or related human services.
  • 1 to 2 years-experience in an outpatient, day treatment, residential and/or hospital setting addressing mental health 
  • Preferred Certified Behavioral Health Case Manager
