Job description

Trusted Hands Senior Care is seeking a compassionate and detail-oriented Patient Care Coordinator to support the delivery of exceptional in-home care. This role serves as the backbone of our care coordination efforts—bridging communication between patients, nurses, caregivers, and community partners. The ideal candidate is highly organized, proactive, and committed to ensuring every client receives the highest quality care through accurate documentation, compliance oversight, and seamless coordination.

Key Responsibilities:

Case Management & Client Coordination

  • Manage all aspects of client files to ensure accuracy, completeness, and compliance with state and local regulations.
  • Maintain and update client records daily—both hard and digital copies—with accurate notes and documentation.
  • Perform initial client intake and onboarding; manage the full cycle of the referral-to-admission process.
  • Build and sustain strong relationships with case managers, social workers, and care coordinators to align with ongoing care expectations.
  • Conduct monthly customer satisfaction survey calls and document feedback for quality improvement.

Care Coordination & Scheduling

  • Collaborate with nursing, billing, and scheduling teams to ensure coordinated, timely patient care.
  • Assist nurses in developing and maintaining supervisory visit schedules (every 90–120 days).
  • Organize and schedule in-person visits with new and existing clients for onboarding, supervisory visits, or training.

Quality Assurance & Compliance

  • Prepare and maintain client files for local and state audits on an ongoing basis.
  • Ensure proper authorizations are obtained and maintained for all clients to support accurate billing and compliance.
  • Manage compliance tracking and reporting; assist with closing compliance gaps.
  • Monitor time-sensitive documentation and follow-up deadlines to maintain regulatory and agency standards.

Communication & Reporting

  • Input and manage all new and existing clients in the appropriate systems and platforms.
  • Maintain real-time updates and notations for proper internal communication and reporting.
  • Check and respond to voicemails and emails throughout the day to maintain efficient communication.
  • Participate in direct client-facing interactions as needed, providing a compassionate and professional presence.
  • Support nurses in the field and participate in direct patient care activities when required.

Qualifications & Skills:

  • Experience in home care, healthcare administration, case management, or nursing coordination preferred.
  • Strong knowledge of HIPAA compliance and healthcare documentation standards.
  • Exceptional organizational, time management, and communication skills.
  • Proficiency with cloud-based platforms, EMR systems, and Microsoft Office/Google Suite.
