Responsibilities
  • Provide direct support and guidance to individuals in the community who are experiencing mental health or substance use challenges
  • Meet clients where they are-at home, in the community, or wherever support is needed
  • Develop and maintain strong, trusting relationships with clients
  • Collaborate with interdisciplinary teams to create person-centered care plans
  • Document interactions, progress, and outcomes in accordance with agency policies and regulations
  • Advocate for clients to ensure access to resources and services
Qualifications
  • Bachelor's degree in human services, social work, or a related field preferred
  • Experience working with individuals experiencing mental health or substance use challenges is highly desirable
  • Excellent communication, problem-solving, and interpersonal skills
  • Ability to work independently in community settings
  • Valid driver's license and reliable transportation required
