 QUALIFICATIONS:

1. Two or more years' experience in the fields of healthcare, public health, social service, or quality improvement.

2. Minimum of 1 year experience in value-based payment programs, population health, or care coordination; a background in medical billing is a plus.

3. Bachelor's degree in business or health related field; a combination of experience and post high school education may be substituted for degree requirements.

4. Knowledge of typical quality and value-based care programs within Medicare, Medicaid, and commercial payors.

5. Knowledge of CMS Quality Metrics. 

6. Excellent organizational skills and strong written and verbal communication skills.

7. Strong computer skills, particularly in Microsoft Excel and Electronic Health Records.

8. Ability to read, interpret, and analyze data.

9. Able to build and maintain effective partnerships internally and externally with an awareness of community resources.

10. Strong analytical skills with the ability to manage and prioritize multiple tasks.

11. Fluent in written and spoken English. Spanish fluency is desirable.

Requirements

  RESPONSIBILITIES:

  1. Responsible for managing the      success of Valley Family Health Care's Value Based Payment Programs. 
  2. Manage the care      coordinators/RN case managers, and the Population Health Specialists to      include directing work, performance management and supervision.
  3. Monthly reporting to care teams and Leadership.
  4. Working closely with Data Analytics team and external      partners to ensure reporting is valid and utilizable. 

5. Maintaining reporting requirements for value-based payment programs.

6. Maintaining relationships with value-based payment programs partners.

7. Maintenance of population health tools. 

8. Quality improvement initiatives and projects related to quality metrics. 

9. Training of care teams related to value based payment programs and clinical quality metrics.

10. Promote clear communication among the integrated care team. 

11. Participate in other VFHC meetings and committees as assigned.

12. Assist with meeting quality measures by completing tasks related to VFHC initiatives.

13. Perform other duties as assigned.


Physical Requirements:

  1. Must be able to lift 25      lbs.
  2. Continuous      sitting, standing, walking.
  3. Correctable      vision and hearing.
  4. The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
  5. The ideal candidate must be able to complete all physical requirements of the job with or without a reasonable accommodation.
