Job description

Indiana Fitness Works, Inc., is seeking a Speech Language Pathologist (SLP) for a position in a long term care setting in Logansport, Indiana at Camelot Care Center.

The SLP will be responsible for evaluation and treatment of communication disorders, swallowing disorders, and cognitive deficits, as prescribed by the physician.

Conduct evaluations, assess home environment (as appropriate), and identify equipment needs relative to speech/language function.
Provides consultation, seeks assistance of others, uses related research and makes recommendations and referrals for the implementation of services.
Develop and implement effective treatment programs.

Identify functional speech defects and establishes a plan of care to improve patient's function.

Implementation and revision of plans of care as necessary.

Provide education/instruction to patient, family members/caregivers, and interdisciplinary team on recommendations for patientโ€™s care.

Prepare and submit notes per agency policy.

Perform other duties as required to facilitate delivery of speech language pathology services.
