ContinueCARE Hospital at Baptist Health - Corbin is seeking a compassionate and skilled Part-Time Physical Therapist (PT) to join our Rehabilitation Services team. In this role, you will provide evidence-based physical therapy to medically complex patients in a Long-Term Acute Care Hospital (LTAC), helping them regain strength, mobility, and independence following serious illness, injury, or surgery.

The Physical Therapist evaluates, plans, and implements individualized treatment programs for adult and geriatric patients while collaborating with physicians, nurses, case managers, respiratory therapists, occupational therapists, and speech therapists to maximize each patient's recovery and quality of life.

Responsibilities
Essential Responsibilities
  • Evaluate adult and geriatric patients and develop individualized treatment plans based on physician orders.
  • Provide physical therapy services designed to restore function, improve mobility, reduce pain, and prevent disability.
  • Perform therapeutic interventions including therapeutic exercise, gait training, transfer training, balance activities, neuromuscular re-education, manual therapy, and functional mobility training.
  • Conduct diagnostic and prognostic assessments of muscle strength, joint mobility, balance, coordination, and functional abilities.
  • Supervise and direct Physical Therapist Assistants (PTAs) in accordance with state practice guidelines.
  • Educate patients and families on home exercise programs, safe mobility techniques, posture, transfers, and the use of assistive devices.
  • Recommend and train patients in the use of walkers, canes, crutches, wheelchairs, braces, and other adaptive equipment.
  • Modify treatment plans based on patient progress and changing clinical needs.
  • Maintain accurate, timely, and complete documentation in the electronic medical record.
  • Participate in interdisciplinary care conferences and discharge planning.
  • Maintain compliance with hospital policies, infection prevention standards, patient safety initiatives, and regulatory requirements.
  • Participate in departmental meetings, continuing education, and performance improvement activities.

Qualifications
QualificationsEducation
  • Bachelor's Degree in Physical Therapy required.
  • Master's or Doctor of Physical Therapy (DPT) preferred.

Experience
  • Experience in an acute care hospital, inpatient rehabilitation, skilled nursing, or Long-Term Acute Care Hospital (LTAC) preferred.
  • New graduates with strong clinical rotations are encouraged to apply.

Licensure & Certifications
  • Current Kentucky Physical Therapist (PT) license in good standing.
  • Basic Life Support (BLS) certification required within 30 days of hire and maintained throughout employment.

Skills
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and patient education abilities.
  • Ability to work collaboratively within an interdisciplinary healthcare team.
  • Strong documentation and organizational skills.
  • Proficiency with electronic medical records (EMR) and basic computer applications.
