At CareTrack, we believe some of the most important care happens between office visits.

Our Clinical Care Coordinators connect directly with patientshelping them stay engaged in their care plan, understand their health trends, and feel supported at home.

This role primarily supports CareTrack's Remote Patient Monitoring (RPM) program, while occasionally assisting with other patient outreach and care coordination initiatives as needed.

A Day in the Life

You begin your day reviewing scheduled patient outreachchecking notes, recent vitals, and identifying opportunities to encourage healthier habits or address concerns.

Your first call may be a patient with elevated blood pressure readings. You discuss trends, reinforce the provider's care plan, answer questions, and encourage continued monitoring.

Later, you connect with a patient who's making progressyou celebrate wins, offer encouragement, and help keep momentum going.

Throughout the day, you may:
