Registered Nurse (RN) – Part-Time Clinical Support & Care Management

Home Careolina Lake Norman Region

Home Careolina is seeking an experienced Registered Nurse (RN) to join our growing team in a flexible, part-time role. This position is ideal for a nurse who enjoys working independently, values flexibility, and has an interest in care management, staff development, and helping older adults remain safe and independent at home.

Position Summary

The RN will provide clinical oversight and support for home care clients, conduct nursing assessments, assist with care planning, provide caregiver education and training, and collaborate with families and community partners.

Responsibilities

· Conduct client nursing assessments and reassessments

· Develop and update individualized care plans

· Provide supervision and clinical support to caregivers

· Participate in caregiver orientation and ongoing staff training

· Assist with quality assurance and clinical documentation review

· Communicate with physicians, families, hospitals, and community providers as needed

· Support client safety, wellness, and independence in the home setting

· Participate in problem-solving and clinical consultation regarding client concerns

· Learn and assist with nurse care management activities as appropriate

Qualifications

· Current North Carolina Registered Nurse (RN) license

· Minimum of 5 years of nursing experience preferred

· Home care, case management, geriatric, rehabilitation, or community health experience preferred

· Strong assessment, communication, and documentation skills

· Ability to work independently and manage a flexible schedule

· Reliable transportation and willingness to travel throughout the Lake Norman and Charlotte area

Preferred Qualifications
