As a Psychiatric Registered Nurse in Home Health, you will deliver professional nursing care directly in patients' homes. You assess their needs, develop personalized care plans, implement treatment strategies, and evaluate outcomes to ensure high-quality, compassionate care for each individual.

Primary Responsibilities:

  • Make initial nursing evaluation visits within 24-48 hours of referral or as directed by the physician
  • Assess and reassess patients' physical condition, mental status, and suicidal potential
  • Initiate and revise treatment plans according to psychiatric nursing standards
  • Instruct and supervise family members in patient care and maintaining a safe environment
  • Provide specialized nursing services and perform physician-ordered treatments
  • Assist families in identifying daily living challenges and accessing community resources
  • Maintain accurate patient records, including medications and treatments
  • Report significant changes in patient condition to physicians and appropriate personnel
  • Provide counseling and crisis intervention to patients and families
  • Participate in agency in-service programs and nursing reports
  • Comply with the Care Management Model and perform 60-day recertification reviews
  • Collaborate with physicians and interdisciplinary teams on patient care
  • Participate in staff conferences and quality assurance/quality improvement processes
  • Perform other duties as assigned

Required Qualifications:

  • Current, unrestricted RN license in state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
  • Recent experience in psychiatric nursing within the last five years OR documentation of psychiatric retraining or continuing education (CEUs)


     

    Must possess one of the following levels of experience and education: 

  • A RN with a Master's degree with a specialty in psychiatric or mental health nursing and licensed in the state where practicing would qualify. The RN must have nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services. 

  • A RN with a Bachelor's degree in nursing and licensed in the state where practicing would qualify. The RN must have 1 year of recent nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program, or other outpatient psychiatric services. 

  • A RN with a Diploma or Associate degree in nursing and licensed in the state where practicing would qualify. The RN must have 2 years of recent nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program, or other outpatient psychiatric services.
