Job Summary:

Responsible for coordinating patient transfers into and out of all hospital acute care facilities while supporting patient logistics and bed flow operations. This role facilitates provider-to-provider communication, capacity coordination, and accurate documentation to ensure timely and appropriate patient placement. In addition, this position supports Emergency Management Communications, serving as a designated coordinator for plain-language overhead emergency announcements and managing emergency notifications through the Everbridge system. The Transfer Center Coordinator functions as a centralized communication resource during routine operations, emergency events, and disaster activations.

Position Responsibilities:

Facilitates timely communication between referring facilities, accepting providers, nursing leadership, bed management, and ancillary departments.

Utilizes the hospital EMR (Electronic Medical Record) system and other clinical information platforms to support patient logistics and transfer coordination.

Coordinates inbound and outbound patient transfers between facilities in compliance with Emergency Medical Treatment and Active Labor Act (EMTALA) and hospital policy.

Serves as a designated coordinator for plain-language overhead emergency announcements: Manages announcements related to medical emergencies, behavioral or security-related incidents, security alerts and threats, tornado warnings and severe weather alerts, facility emergencies, and internal disaster notifications.

Maintains confidentiality and professionalism when handling sensitive patient and emergency information.

Maintains situational awareness during emergency events and escalates issues to Emergency Management/ Clinical leadership as appropriate.

Position Qualifications Required:

Required Experience:

1-3 years of experience in patient logistics, transfer coordination, hospital operations, emergency communications.

Required Education:

High School graduate or equivalent.

Graduate of a recognized Emergency Medical Technician-Basic Training Program preferred.

Training / Certification / Licensure:

Experience using hospital EMR systems preferred.

Experience using Everbridge or similar mass notification systems preferred.
