Licensed Practical Nurse (LPN-W2) Legend DDD Services – Mesa, AZ
Full-Time & Part-Time Positions Available Make a meaningful impact, in a supportive, team-driven environment.
Legend DDD Services is seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to support adults with developmental disabilities in our Nursing-Supported Group Homes. Our LPNs play a vital role in ensuring the health, safety, and well-being of the individuals we serve. Position Summary The LPN provides direct nursing care, medication oversight, health monitoring, and clinical support to residents in DDD group home settings. This role works closely with the LPN Coordinator, DSPs, and leadership to maintain compliance, deliver high-quality care, and support residents with dignity and respect. Scheduling Options We offer flexible scheduling ,with opportunities for additional hours.

  • Full-time and part-time shifts available
  • Requirement for 1 weekend swing shift per month

Responsibilities

  • Provide nursing care and health monitoring for residents in group home settings
  • Administer medications and ensure accurate MAR documentation
  • Complete routine assessments and report changes in condition
  • Support DSPs with health-related tasks and training as needed
  • Assist with ADL’s alongside the DSPs on staff
  • Maintain compliance with DDD, HCBS, and agency nursing standards
  • Communicate effectively with the LPN Coordinator, Managers, and clinical leadership
  • Assist with care plans, follow-up instructions, and physician orders
  • Ensure a safe, clean, and supportive environment for all residents
  • Participate in monthly nursing reviews and audits as required
