Legend DDD Services is seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to support adults with developmental disabilities in our Nursing-Supported Group Homes. Our LPNs play a vital role in ensuring the health, safety, and well-being of the individuals we serve. Position Summary The LPN provides direct nursing care, medication oversight, health monitoring, and clinical support to residents in DDD group home settings. This role works closely with the LPN Coordinator, DSPs, and leadership to maintain compliance, deliver high-quality care, and support residents with dignity and respect. Scheduling Options We offer flexible scheduling ,with opportunities for additional hours.Â
- Full-time and part-time shifts availableÂ
- Requirement for 1 weekend swing shift per monthÂ
ResponsibilitiesÂ
- Provide nursing care and health monitoring for residents in group home settingsÂ
- Administer medications and ensure accurate MAR documentationÂ
- Complete routine assessments and report changes in conditionÂ
- Support DSPs with health-related tasks and training as neededÂ
- Assist with ADL's alongside the DSPs on staffÂ
- Maintain compliance with DDD, HCBS, and agency nursing standardsÂ
- Communicate effectively with the LPN Coordinator, Managers, and clinical leadershipÂ
- Assist with care plans, follow-up instructions, and physician ordersÂ
- Ensure a safe, clean, and supportive environment for all residentsÂ
- Participate in monthly nursing reviews and audits as requiredÂ

Requirements

- Active Arizona LPN license in good standingÂ
- Valid driver's license and reliable transportationÂ
- Ability to obtain or maintain:Â
- Level One Fingerprint Clearance CardÂ
- CPR/First AidÂ
- Article 9Â
- Strong clinical judgment and communication skillsÂ
- Ability to work independently and as part of a teamÂ
- Dependable and able to maintain consistent attendanceÂ
