Job Description
Vista Prairie Communities at Keelson Harbour is seeking a part-time LPN.
MAJOR DUTIES & RESPONSIBILITIES
ADMINISTRATIVE FUNCTIONS
  • Works under the direction of the RN to monitor clients' needs and changes in condition, conducting focused assessments when appropriate and reporting the results to the RN
  • Ensures that unlicensed staff members perform their duties and responsibilities are accomplished in a timely, quality manner and reports problems to RN.
  • Reports changes or problems in resident conditions or other concerns about clients to the RN in a timely manner.
  • Initiates emergency treatment and reports to the RN.
  • Reviews client care plans developed by the RN and consults with RN about resident conditions and their treatments.
  • Assists with implementation of physician orders.
  • Ensures accuracy and promptness in transcribing physician orders.
  • Carries out proper charting practices and documentation procedures.
  • Assembles and operates equipment such as oxygen suppliers and other equipment used for resident care.
  • Orders, sets up and administers medications.
  • Assists RN in orienting, training and mentoring new staff.
  • Attends required meetings, in-service training programs, and conferences.
  • Performs delegated tasks and other duties as assigned by the RN.
  • Assists the RN in the monitoring and evaluation of the job performance of unlicensed staff.
  • Consults with RN regarding unlicensed staff performance.
  • Plans, assigns, and directs the work of unlicensed staff within established guidelines.
  • Assists unlicensed staff as necessary in performing their duties.

ADMISSION, TRANSFER, AND DISCHARGE FUNCTIONS
  • Assists with startup of services for new resident transfers to settings such as hospitals, nursing homes or assisted living and discharges as requested by RN supervisor.

WORK ENVIRONMENT
  • Primary responsibilities will be conducted in clean, well-lit, climate controlled areas such as office space, resident rooms, hallways, work rooms, and resident dining area and meeting rooms.
  • The noise is usually moderate.
  • Is subject to exposure to blood-borne pathogens and infectious diseases.
  • Is subject to exposure to chemical and respiratory hazards.
WORKING CONDITIONS
  • Works in all areas of the communities.
  • Is involved with residents, personnel, families, visitors, etc. under all conditions and circumstances.
  • Communicates frequently with other personnel.
  • Works beyond normal working hours, on weekends and holidays, and in other positions when necessary.
  • Attends and participates in continuing education programs.

PHYSICAL AND SENSORY REQUIREMENTS (WITH OR WITHOUT THE AID OF MECHANICAL DEVICES)
  • Must be able to move intermittently throughout the working day, must be able to stand, stoop, walk, kneel and crouch.
  • Requires good hand-eye coordination, arm, hand and finger dexterity, including ability to grasp and visual acuity to use a keyboard, operate equipment and read.
  • Frequently required to sit, reach with hands and arms, talk and hear.
  • Must be in good general health and demonstrate emotional stability.
  • Must be able to lift, push, pull, and move a minimum of 20 pounds.
  • Must be able to assist in the evacuation of residents.

Qualifications
EDUCATION AND EXPERIENCE
Must be registered and licensed with the State of Iowa as an LPN, and must maintain current unencumbered license with the Board of Nursing.
SPECIFIC REQUIREMENTS
  • Working knowledge of state, federal and local regulations.
  • Must be knowledgeable about validation techniques, reality orientation and/or appropriate diversionary activities for clients with dementia.
  • Must receive a "not disqualified" criminal background result from Department of Human Services and may not be on the OIG exclusions list.
  • Must be a minimum of twenty-one (21) years of age.

REQUIRED SKILLS AND ABILITIES
  • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing service practices.
  • Must possess a desire to work with aged, infirm, and handicapped persons.
  • Must be knowledgeable on current policies and procedures; including the ability to write procedures.
  • Must be able to relate to and work with the elderly, emotionally upset, and, at times, hostile people, dementia residents and occasionally challenging behaviors.
  • Excellent interpersonal skills and demonstrated ability to connect and communicate effectively with diverse populations.
  • Ability to work in a fast paced environment and to prioritize, organize and manage multiple priorities.
  • Ability to work beyond normal working hours, on weekends and holidays, and in other positions, when necessary.
  • Some travel to meetings and seminars required.
  • Attends and participates in continuing education programs.
