<p><b>Physical Med and Rehab Physician (PM&R) / Medical Director Green Bay WI</b></p> <p><b>Full time or Part time</b></p> <p> </p> <p>We are looking for two experienced Physician to join our practice part time or full time in the Green Bay area as well as a having a second need in our Sheboygan/Manitowoc Area, just one hour south of Green Bay. We are seeking a compassionate provider to be our go-to person for ensuring the best patient care possible in our Rehab Hospital/care facilities. The physician is responsible for diagnosing and treating patients in a patient centered compassionate environment as a part of a cohesive interdisciplinary clinical team. </span>Great place to raise a family, and easy access to metropolitan culture!</p> <p><span style="background:white"><b>Responsibilities:</b></span></p> <ul> <li><span style="background:white"><span style="tab-stops:list .5in">History and Physical Exams- Examine patients and their medical records, pertinent documents/imaging/labs/test</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Order and study diagnostic tests (blood tests, X-rays etc.)</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Review and approve rehabilitation care plans as needed </span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Actively participate in interdisciplinary team and patient care conferences </span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Diagnosis, management and treatment of acute episodic illnesses and stable chronic conditions, evaluation of current therapies, health maintenance, health promotion, illness prevention education and counseling, and management of urgent care matters</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Prescribe medications</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Write referrals and consult other physicians for comprehensive patient care</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Maintain accurate records and schedules- Establishes and maintains a system for tracking and coordinating the designated patient caseload and their progress throughout the entire acute or chronic episode of care</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Inform patients about their health conditions and prescribed medications</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Educate patients and their family members on diseases, epidemics and prevention- teaching and counseling to patients and families</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Collaboration with the healthcare team- Monitors, evaluates, and communicates relevant data regarding clinical problems consulting physicians and appropriate members of the interdisciplinary team</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Oversee patients’ overall health- identify opportunities for each patient's needs relative to his/her age, Monitors progress of patients and collaborates with other team members to ensure continuity of care.</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Identifies and addresses potential deterrents to compliance with treatment regime.</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Advance professional knowledge and conduct research</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Follows up with patients if they are discharged to assess if needs are being met and determines if additional follow-up care is required.</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Assure the provision of patient services while on duty</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Collaborate closely with the leadership regarding operational policies and procedures that may impact patient care</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Attend monthly meetings as required</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Review and approve rehabilitation care plans as needed and actively participate in interdisciplinary team and patient care conferences </span></span></li> </ul> <p><span style="background:white"><b>Skills:</b></span></p> <ul> <li><span style="background:white"><span style="tab-stops:list .5in">Valid Medical license in WI</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">MD/DO BCBE in Physical Medicine and Rehab (PM&R) / Physiatry </span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">ACLS Certified</span></span></li> </ul> <p><b>Schedule:</b></p> <ul> <li><span style="background:white">Full time or Part time </span></li> <li><span style="background:white">Flexible shifts</span></li> </ul> <p><span style="background:white"><b>Salary (range):</b></span></p> <ul> <li><span style="tab-stops:list .5in">$175.00 -
