Job Title: Part-Time RN Case Manager

Company: Athena Hospice of Rhode Island

Job Summary:

Athena Hospice of Rhode Island is seeking a part-time RN Case Manager to join our dedicated team! As a Hospice RN Case Manager, you will be responsible for planning, organizing, and directing the hospice care plan for assigned patients under the direction of the clinical director. We offer a competitive hourly rate of $45 to $50, with weekly pay, and travel throughout Rhode Island is required.

Responsibilities:

  • Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs

  • Provide a complete physical assessment and history of current and previous illnesses

  • Supervise hospice aides

  • Provide professional nursing care by utilizing all elements of the nursing process

  • Assess and evaluate patient status, writing and initiating a plan of care, and regularly re-evaluating patient and family/caregiver needs

  • Develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions

  • Provide counseling to patients and families in meeting nursing and related needs

Requirements:

  • Active and unencumbered RN license in Rhode Island

  • Nursing degree from an accredited college, university, or school

  • Prior experience in hospice is preferred
