Responsibilities
  • Conduct comprehensive patient assessments and document symptoms, progress, and outcomes accurately.
  • Perform initial home health evaluations and ongoing reassessments as needed.
  • Develop and implement individualized plans of care.
  • Complete OASIS documentation accurately and in a timely manner.
  • Supervise Home Health Aides (HHAs) and provide clinical guidance as appropriate.
