Role Overview
We're seeking a Part-Time Primary Care NP to provide on-site care to Assisted Living/RCFE residents across Bakersfield and Terra Bella.
What You'll Do
  • Routine & urgent assessments; chronic disease management
  • Post-discharge follow-ups, med reconciliation, labs/diagnostics
  • Coordinate with facility staff, specialists, families/POAs
  • Same-day documentation; CMS/regulatory compliance

Schedule & Setting
Part-time position - 3 days per week delivering on-site care across Bakersfield and Terra Bella with strategically clustered routes designed to minimize travel and maximize time with patients.
