Overview:

Applesoft Homecare Services LLC is seeking a compassionate and highly skilled Registered Nurse (RN) to provide expert in-home healthcare services to our patients. This position involves delivering skilled nursing care, with a particular focus on wound care, PICC line care, and trach care. The role offers competitive per-visit rates, flexible scheduling, and mileage reimbursement.

Key Responsibilities:

  • Perform comprehensive assessments of patients' clinical conditions, focusing on wound care, PICC line care, and trach care.
  • Provide advanced wound care, including debridement, dressing changes, and infection prevention.
  • Manage and maintain PICC lines, including flushing, dressing changes, and monitoring for complications or infection.
  • Deliver trach care, including suctioning, monitoring for respiratory issues, and changing tubes as needed.
  • Develop and implement individualized care plans, in collaboration with the interdisciplinary team.
  • Educate patients and their families on wound care, trach care, and other medical procedures to promote self-care.
  • Monitor patient progress and make necessary adjustments to care plans, reporting significant changes to the supervising physician.
  • Maintain accurate and thorough documentation of patient care activities in compliance with Applesoft Homecare Services policies and healthcare regulations.
  • Ensure adherence to safety, infection control, and quality care standards.
  • Communicate regularly with healthcare team members to ensure a coordinated and holistic approach to patient care.
  • Provide compassionate, patient-centered care that promotes dignity and respect.

Qualifications:

  • Active RN license in the state of Texas.
  • Minimum of 2 years of clinical experience, preferably in home health or acute care, with specialized experience in wound care, PICC line care, and trach care.
  • Current CPR/BLS certification.
  • Valid drivers license and reliable transportation.
  • Strong assessment, critical thinking, and organizational skills.
