The Registered Practical Nurse (RPN) reports to the Director of Care and is expected to coordinate and oversee all aspects of resident-focused care delivery. A Registered Practical Nurse should also promote the values, goals, and philosophy of the Home.

The Registered Practical Nurse (RPN) reports to the Director of Care and is expected to

coordinate and oversee all aspects of resident focused delivery of care. A Registered Practical

Nurses should also promote the values, goals, and philosophy of the Home.

Responsibilities:

  • Development and modification of the care plans in response to residents’ individualized

health care needs, change in health status

  • Liaises with other health care personnel, agencies and families
  • Maintains good relationships with team members, residents, families and the general public
  • Engages in many meaningful interactions with residents during your entire shift
  • Supervises all activities on the unit, takes charge as required
  • Monitors performance and completes performance evaluations for the PSWs
  • Directs, assigns and evaluates the work of the PSW’s
  • May be in charge of Home as required. Utilizes leadership skills to identify problems and

problem- solving on the unit

  • Monitors that care is consistently delivered with the resident Bill of Rights.
  • Required to understand the nature and meaning of quality indicators used by the Home

and to understand which indicators are publicly reported

  • Required to participate in the improvement of the indicators and achieve satisfactory

results

  • Actively listens to resident and family concerns and takes appropriate action to resolve

the issue in accordance with the policies of the Home.

  • Transcribes and process physician's orders for medication or other treatment in

accordance with CNO standards

  • Orders and ensures nursing supplies and equipment are properly and efficiently used
  • Conducts unit rounds at the beginning and end of shift
  • Administers medications and treatments in compliance with the policies of Pharmacy

provider, the Home and the Standard of the College of Nurses. Documents and reports

all errors and omissions

  • Completes and ensures residents' clinical care assessments, care plans, and

documentation are accurate, and up to date in accordance with CNO standards and

changes communicated to interdisciplinary team, residents, and SDM/POAs as required

  • Attends and participates in resident care conferences as required.
  • Maintains accurate and complete records of nursing observations and care in

compliance with the legislative requirements and the policies of the Home

  • Assesses changes in health status and reports changes to the Substitute Decision Maker

(SDM) and interdisciplinary team

Registered Practical Nurse (RPN) Hiring Package 2

  • Completes assessments (RAI-MDS and other assessments) develops and updates plan

of care for each resident in collaboration with the resident and/or SDM and the

interdisciplinary team

  • Inspires others to be motivated and work with passion, and empowers team members
  • Any other tasks as assigned

Qualifications:

The qualifications needed to join our family are as follows:

  • Current Certificate of Competence with the College of Nurses of Ontario as Registered

Practical Nurse or a Registered Practical Nurse registered with a regulatory authority in

a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of

a certificate of registration authorizing independent practice in Ontario as of right

Exemption Bill 60

  • Basic CPR an asset
  • Experience in long-term care setting an asset
  • Ability to work with and relate to interdisciplinary team members
  • Ability to work from the heart and make others smile
  • Well-developed written and oral communication skills
  • Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one

(1) year of being hired

  • Must have a clear vulnerable sector screen, no older than 6 months, TB test results less

than 6 months old, must be able to provide three supervisory reference
