Your Role
  • Provide skilled nursing care and manage symptoms in the patient's home

  • Develop individualized care plans with the interdisciplinary team

  • Communicate and educate patients and families

What You Bring
  • Current Arizona RN license

  • At least 1 year of nursing experience (hospice/palliative care preferred)

  • Associate's or Bachelor's degree in Nursing

  • CPR certification, valid driver's license, reliable vehicle, and insurance
