Community East has a part-time RN Case Manager position available in the Emergency Department. The Registered Nurse Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services. Responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family. To provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and management and discharge planning. Has accountability for the care, coordination, and discharge planning of inpatient and observation services.
Exceptional Skills and Qualifications

  • Graduate of National League for Nursing, Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education accredited school of nursing. 
  • Licensed as a Registered Nurse by the Indiana Professional Licensing Agency - (required).
  • Two years of recent clinical experience or three years recent hospital case management experience- (required).
  • Certification in Case Management (CCM or ACMA) - (preferred).
  • Team building skills with the ability to function independently and interdependently as a member of a network team.
  • Demonstrates the ability to be flexible in work schedules and coverage at any site.
  • Strong organizational and problem-solving skills.
  • Exhibits openness to change and willingness to try new approaches to problem solving.
  • Can establish rapport with a wide variety of people.
  • Strong computer skills.
