Job description

Description:

The Registered Nurse (RN) is the health care professional who will work with clients and their families to assess the healthcare needs, education and support required to reach and or maintain the client’s optimum health status. The RN will ensure that all clients receive the highest quality of care which will enable them to remain in their home and in their community of choice.

  • Ensure compliance with all standards and guidelines established by the State Board of Nursing and Department of Health for the state in which they are practicing as well as that of A First Name Basis (AFNB).
  • Perform all in home client assessments within the guidelines of the respective governing bodies and AFNB.
  • Determine if AFNB is capable of providing appropriate services to meet the medical needs of potential and / or current clients.
  • Perform assessment on each client that is identified as in need of services at least annually, and or following any significant health changing events.
  • Complete and provide the required documentation ensuring that the Caregiver has met all training and competency requirements to adequately perform identified non-complex tasks.
  • Document all necessary aspects of client care and Caregiver interactions to ensure compliance with all guidelines are followed, including but not limited to cross department collaboration
  • Performs other incidental and related duties as required and assigned.





Requirements:
  • Must have a valid State RN License in the state in which they are working.
  • One year experience in home care, health care or senior-related industry required, preferred.
  • Knowledge or experience working of payroll systems and processes.
  • High school diploma or GED required.
  • Office/administrative support skills.
  • Excellent written and verbal communication skills.
  • Must have consistent, reliable transportation to commute to client homes within territory.
  • Ability to work in a fast paced, multi-task environment with competing priorities.
