Dependable Nursing is seeking an experienced, compassionate RN Case Manager to coordinate and oversee skilled home health care for patients across the lifespan โ€” from pediatrics through senior care. This role begins as a part-time, hourly position and is expected to ramp quickly to full-time as the territory grows. The ideal candidate is a confident, self-directed clinician who is proficient in OASIS, thrives in the field, and is excited to help build a growing home health program.


This position starts part-time on an hourly basis, with the schedule built around current caseload in the territory. As the patient census grows, the role is expected to transition to full-time. This is an excellent opportunity for an RN who wants to grow with an expanding home health program and help shape case management in your region in south and southeast San Diego as the team scales.


Key Responsibilities

  • Conduct comprehensive in-home assessments and complete accurate, timely OASIS documentation at start of care, recertification, resumption of care, transfer, and discharge.
  • Complete start-of-care visits and assessments as assigned โ€” including, when required and permitted under applicable regulations, on therapy-only (PT/OT) cases.
  • Develop, implement, and update individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary team.
  • Provide skilled nursing care and case management for a caseload spanning pediatric, adult, and geriatric patients.
  • Coordinate care across disciplines (PT, OT, ST, HHA, MSW) and ensure services are delivered per the plan of care.
  • Supervise LVNs and home health aides, including required supervisory visits.
  • Educate patients and caregivers on disease management, medications, and care plans to promote safety and independence in the home.
  • Monitor patient progress, adjust care plans as needed, and communicate changes to physicians and the care team.
  • Ensure documentation and care delivery comply with Medicare, Medi-Cal, payer, and regulatory requirements.
  • Participate in care conferences, case review meetings, and quality improvement initiatives.
Requirements:
  • Active, unrestricted California RN license in good standing.
  • Proficiency in OASIS documentation is required.
  • Must understand Medicare, VA and Tricare.
  • Experience caring for patients across the age spectrum โ€” pediatric through senior care โ€” is required.
  • Home health experience, with working knowledge of the plan-of-care process and home health regulations.
  • Current BLS/CPR certification.
  • Valid California driverโ€™s license, reliable transportation, and auto insurance (this is a field-based role).
  • Strong clinical assessment, organizational, and documentation skills.
  • Ability to work independently and manage a caseload across the assigned territory.

Preferred Qualifications

  • Experience with electronic medical records (e.g., Axxess).
  • Bilingual (English/Spanish) a plus, given the territory served.
  • Familiarity with Medi-Cal, Schools, Regional Centers, and self-directed care programs.
