Seniority Healthcare is seeking a part-time Nurse Practitioner or Physician Assistant to join our team in New City, NY for 16 hours a week. In this role, you will provide comprehensive care to a high-need, high-risk geriatric population. The ideal candidate has a background in Internal Medicine, Family Medicine, or Geriatrics, and can work independently while collaborating with a multidisciplinary team. This position offers a balance of direct patient care and remote responsibilities—such as prescription refills, lab reviews, and patient communication—allowing for a flexible and rewarding schedule.

Key Responsibilities

  • Conduct thorough physical exams and wellness assessments for seniors
  • Follow up on care plans to ensure the best possible outcomes
  • Propose and manage treatment plans for chronic and acute conditions
  • Maintain accurate patient records and manage appointments effectively
