Job description


Care2U is a fast-growing healthcare startup providing on-demand acute care and hospital-level care in the patient's own home throughout New York City, Nassau, and Westchester counties. Our mission is to reduce unnecessary ED visits and hospitalizations by delivering high-quality care where patients feel most comfortable - at home.
*This is a remote telehealth position. However, initial training will be conducted in person in Plainview, NY. Candidates must hold a valid New York medical license.
Job Responsibilities:
  • Conduct patient assessments via telehealth with the assistance of on scene PAs, NPs or Paramedics to manage patients with acute illnesses and injuries.
  • Provide treatment and clinical management consistent with evidence-based guidelines.
  • Respond to on-demand and scheduled calls and focus on quality and efficiency metrics to improve patient experience.
  • Work collaboratively with our multi-disciplinary team of field providers and care coordinators to efficiently manage complex care needs in the home.
  • Assesses patients for risk factors associated with the care of the acute and complex chronically ill patient including: medication compliance and side effects, limited mobility, impaired nutrition, fall safety, and social isolation.
  • Complete patient records in the Electronic Health Record in a timely manner that meets acute care compliance standards and captures patient acuity.
  • Maintain a safe, secure, equitable and healthy work environment by following safety procedures, quality standards and comply with regulations.
  • Maintain patient confidentiality
  • Stay current on in-home acute care literature, technology, and regulatory issues.

Job Type: Part-Time
Hourly Rate: $140 (up to $170 with volume bonuses)
Supplemental pay types: Volume Incentives
Schedule: Minimum 80 hours/month, including 20 weekend hours

Job Qualifications:
  • Medical Degree from an accredited university.
  • Board Certified or Board Eligible in Emergency Medicine, Internal Medicine or Family Medicine.
  • Valid Medical License in New York State.
  • Able to meet credentialing requirements, including DEA certificate
  • Emergency Department or Hospitalist Experience
  • Experience in Geriatric Medicine, Hospital at Home, or Post-Acute Care is a plus.
  • Experience providing telehealth or in-home visits a plus.
  • Additional experience in geriatric medicine, hospital at home, or post-acute care a plus.
